Town of Greeneville FT employees may obtain a copy of their 1095-C form by logging into their account on www.mybenefitschannel.com and going to “Online Pay Info”, then down to “1095-C”, and then click the year “2025”.  Your 1095-C form will then be displayed on the screen for you to print.

Employees can also request a copy of their 1095-C form by contacting the HR department in one of the following ways:
